Title: Director of Population Health
Department: Clinic Population Health
Reporting Relationships: Executive Director Quality
Job SummaryThe Director of Population Health manages, oversees, develops, coordinates and implements work to meet quality program requirements and ensure deadlines and deliverables are met. Additionally, the director develops, analyzes, researches, and coordinates quality program data to ensure the accuracy of shared data. The director also creates and manages new workflows to improve coordination of care. In addition, the director oversees assigned team members' work to ensure desired outcomes are achieved.
Job Duties- Oversee the Clinic Care Coordinator Supervisor and care coordinators, including their work to meet clinic quality program requirements and deliverables.
- Oversee the Clinic Quality Manager and monthly clinic quality program reporting.
- Manage, develop, and coordinate work to meet MIPS quality program requirements and implement workflows to improve outcomes and overall score.
- Manage and implement workflows and organizational requirements, deadlines, and deliverables in applicable clinic quality programs.
- Manage and implement workflows and organizational requirements, deadlines, and deliverables in the Accountable Care Organization (ACO) to ensure program requirements are met.
- Develop, coordinate, and implement education related to quality measures and program work.
- Develop and implement improved workflows to increase quality program outcomes and identify and implement revenue-based patient care within population health management.
- Coordinate quality program meetings and develop agendas accordingly.
- Develop and implement a Community Health Needs Assessment (CHNA) per required guidelines, rules, and regulations.
- Research inventive methods to improve patient care and quality outcomes, implementing strategies as identified and approved.
- Perform other duties as assigned by supervision.
Education:
Required: Registered Nursing Licensure
Desired: BSN, MSN, or other advanced degrees
Training and Experience:
- A minimum of 3 years in quality improvement and program work.
- A minimum of 2 years in a leadership and management position.
- Microsoft Office skills, including Word, Excel, PPT, and virtual tools with the ability to utilize these tools for scheduling meetings, develop reports and presentations as needed.
Desired:
- Knowledge and application of one or more skillsets, including Lean Six Sigma, Motivational Interviewing, Appreciative Inquiry Model, or other professional skill.
- Experience in adult learning or education.
Job Knowledge:
- Electronic Health Records and the basic methods of running quality performance reports.
- Centers for Medicaid and Medicare (CMS) websites including access requirements for various site related to various quality programs.
Desired:
- Knowledge of coding and risk stratification methods of various payers.
- Must be able to report to work fit for duty and free of any adverse effects of illegal drugs, medical marijuana, prescription medication, and/or alcohol.
- Must be able to effectively communicate both orally and in writing with other individuals.
