Physical Therapist
Location: Woodrow Wilson Keeble Memorial Health Care Center (WWKMHCC), Sisseton, South DakotaSchedule: MondayFriday, 8:00 a.m. 4:30 p.m. (No weekends or holidays)
Position OverviewThe Physical Therapist will provide comprehensive outpatient physical therapy services at WWKMHCC to support the healthcare needs of approximately 7,000 tribal members on the Lake Traverse Reservation. Services will include evaluation, treatment planning, implementation, and patient/family education in accordance with IHS and WWKMHCC standards of care. This is a non-personal services contract position requiring credentialing, privileging, and compliance with federal regulations.
Duties & Responsibilities-Evaluate, diagnose, and provide rehabilitation services for patients with acute and chronic conditions, including musculoskeletal, neurological, and post-surgical cases.-Develop individualized treatment programs and involve patients and families in goal setting.-Provide ongoing assessments and modify treatment plans as necessary.-Consult and collaborate with referring physicians regarding patient progress.-Educate patients and families on home treatment and preventative care.-Document all patient care activities in the Electronic Health Record (EHR) within established timeframes.-Participate in Patient Centered Medical Home (PCMH) model of care.-Attend and contribute to staff meetings, committees (Quality Assurance, Pharmacy & Therapeutics, etc.), and peer reviews as assigned.-Comply with HIPAA, CMS, IHS, and WWKMHCC bylaws, policies, and regulations.-Maintain professional appearance and uphold cultural sensitivity standards in interactions with Native American patients and community.-Complete all orientation, credentialing, privileging, and background clearance requirements before beginning clinical duties.
Qualifications
Education: Graduate of an accredited Physical Therapy program.Licensure: Active, unrestricted Physical Therapy license in at least one U.S. state or territory.Experience: Minimum of one (1) year of clinical physical therapy experience within the past three (3) years.Certifications:-Current Basic Life Support (BLS) certification (required).-Continuing education evidence required.Technical Skills: Proficiency in Electronic Health Record (EHR) documentation and computer literacy.Other Requirements:-Must be fluent in English (reading, writing, and speaking).-Must provide proof of immunizations (Hep B, MMR, Varicella, Tdap, Flu, COVID) and a current negative PPD.-Must pass FBI fingerprint check, Tier 2 background investigation, and comply with HSPD-12 credentialing.-Must not appear on the OIG Exclusion List.
