Description

Job Title: Clinical Pharmacist Specialist
Department: Oncology
Location: Rochester General Hospital
Hours Per Week: 40 hours
Schedule: Monday-Friday day shift; On-call, every 6th weekend
SUMMARY:
The Clinical Pharmacist Specialist provides clinical pharmacy services for the Lipson Cancer Institute in a professional and competent manner to support the inpatient malignant hematology service along with other inpatient hematology and oncology patients, including treatments such as BiTE (bispecific T-cell engager) therapies.

RESPONSIBILITIES:

  • Clinical Services:
    •Possess a broad scope and depth of pharmacotherapy knowledge and clinical skills. Must be able to perform independently and be responsible for complex decision-making, such as drug therapy selection.

    • Participates in weekday inpatient multidisciplinary rounds with malignant hematology and BiTE therapy services.

    •Pharmacist ownership of preparing, releasing and coordinating chemotherapy and other anti-cancer orders for inpatient administration using Beacon.

    •Patient/caregiver medication education: new start chemotherapy and other anti-cancer therapy, transitions of care counseling, coordinate weekday meds-to-beds for inpatients.

    •Able to develop, recommend, and monitor therapeutic plans.

    •Participates in the Oncology Pharmacy & Therapeutics committee as well as the System Pharmacy & Therapeutics Committee meetings in a leadership role.
    •Function on Health System multidisciplinary teams in the development of protocols, order sets, and critical pathways to optimize patient care.
    •Develop, conduct, and participate in drug quality programs, drug usage evaluations, and research. Present findings at appropriate local or national professional meetings.
    •Able to respond to complex drug information requests using literature searching expertise. Evaluate and summarize primary literature and incorporate the information into formulary reviews and patient care decisions.
    • Works with pharmacy management and specialty departments to implement policies and procedures to ensure compliance with all applicable regulations (for example: state and federal law, accrediting bodies and National Patient Safety Goals).

    • Occasionally rotate into the RGH Lipson Cancer Center Outpatient Infusion Pharmacy to assist in operational duties.

    •Detect, prevent, monitor, document, and report adverse drug events and medication errors.
    • Respond to patient care urgencies/emergencies within your area of expertise.

    Education:
    •Take a leadership role in teaching prescribers, pharmacists, and/or nursing staff throughout the Health Care System through formal teaching programs and informal meetings.
    •Precept Pharmacy Interns and Residents as appropriate.
    •Committed to continual learning of new knowledge and the enhancement of critical thinking and problem-solving skills. Attend at least 1 national professional meeting annually.
    •Functions as the oncology pharmacy lead for PGY1 Residency at Rochester General.

    •Experience in developing PGY1 or PGY2 Curriculum and Resident Precepting.

REQUIRED QUALIFICATIONS:

  • Completion of residency OR fellowship training OR board certification through the Board of Pharmaceutical Specialties. The employee may be working towards board certification as part of a timeline mutually agreed upon by the employee and supervisor.

  • Licensure as a Pharmacist in New York State.

PREFERRED QUALIFICATIONS:

  • Epic and Beacon experience

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $130,000.00 - $168,000.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
