We’re searching for a dedicated Registered Nurse to join our team and help support our patients’ health, comfort, and independence. • Conduct thorough nursing assessments in patient homes, evaluating their full spectrum of health and support needs

• Complete initial OASIS assessments and other required visits such as recertification and discharge, and collaborate with LPNs for ongoing care

• Develop and update personalized care plans in partnership with the care team and families

• Deliver skilled care including wound care, medication management, IV therapy, catheter care, and more

• Monitor patient progress and adjust care as needed while keeping the care team informed

• Provide education to patients and caregivers to empower them in their journey to wellness

• Coordinate with therapists, aides, social workers, and community resources to ensure well-rounded support

• Keep clear, compliant, and timely documentation in our electronic health record system

• Offer emotional support and guidance to patients and their families during difficult times

• Follow infection control and safety protocols to ensure well-being for all involved

• Take part in case reviews, care planning, and performance improvement efforts

• Current RN license in Virginia (LPN experience may also be considered)

• 2 to 3 years of nursing experience, ideally in home health or community care

• Strong clinical judgment and proficiency with skilled nursing tasks

• Excellent communication and empathy to connect with diverse patients and families

• Familiarity with EHR systems and confident documentation skills

• Experience in wound care and diagnosis strongly preferred

• Valid driver’s license and reliable transportation to travel locally

• A passion for providing compassionate, high-quality, and patient-centered care
