We are seeking a Part-Time/PRN Physical Therapist Assistant (PTA) to join our home health team serving the Lake County, IN area. What You'll Do

• Provide in-home physical therapy treatments under the supervising PT's plan of care
• Teach patients and caregivers proper exercises, mobility techniques, and safety strategies
• Monitor and document patient progress accurately and timely
• Communicate updates to the supervising Physical Therapist and interdisciplinary team
• Deliver care in accordance with state, federal, and professional standards
• Promote patient safety and maintain compliance with agency policies
• Participate in interdisciplinary collaboration as needed

What We're Looking For

• Licensed Physical Therapist Assistant (PTA) in the state of Indiana
• Graduate of an accredited PTA program (APTA/CAPTE)
• Minimum 6 months PTA experience (home health preferred, not required)
• Strong communication and documentation skills
• Reliable transportation, valid driver’s license, and required auto insurance
• Compassionate, dependable, and patient-focused approach
