Position SummaryThe Program Supervisor for CalAIM Enhanced Care Management (ECM) and Community Supports (CS) leads a team of case managers delivering whole-person care to Medi-Cal members with complex medical, behavioral health, and social needs. This role is hands-on leadership: the Supervisor is in the work, running 1:1s, reviewing charts, coaching staff in the field, not managing from a distance.
The Supervisor ensures services are delivered in accordance with CalAIM program requirements, IEHP and Molina managed care plan guidelines, and organizational standards for quality and ethics. The role carries direct accountability for the team's documentation quality, audit performance, supervision cadence, staff development, caseload operations, and day-to-day program execution. Performance is evaluated against the Simple Solutions Supervisor Evaluation Scorecard, a 100-point framework supplemented by quarterly anonymous 360° feedback from direct reports.
Staff Supervision and Documentation Oversight
• Hold consistent 1:1 supervision with each direct report at the required cadence.
• Review staff documentation regularly with specific, actionable feedback.
• Complete staff scorecards at the required frequency (weekly light, monthly full, quarterly review).
• Identify, escalate, and address auto-fail triggers on staff scorecards promptly.
• Integrate clinical oversight where required, including RN review of ECM care plans and IHSP review for CS Housing.
Leadership, Coaching, and Staff Development
• Provide timely, constructive feedback that drives measurable performance improvement.
• Address underperformance proactively — initiate Performance Improvement Plans when warranted, rather than delaying.
• Support professional growth through training opportunities, certifications, and career pathing.
• Recognize good work meaningfully and cultivate a supportive, accountable team culture.
• Maintain staff retention at or above agency benchmarks.
Operations and Caseload Management
• Manage caseload distribution equitably across the team and adjust for member acuity.
• Ensure member engagement and retention rates meet program targets.
• Actively manage and track referral pipelines (hospital, SNF, community partners, internal).
• Maintain staff scheduling, time-off coverage, and on-call arrangements with no member coverage gaps.
Care Coordination Leadership
• Ensure case managers complete comprehensive assessments addressing medical, behavioral health, and social determinants of health.
• Review individualized Care Plans to ensure services are person-centered and aligned with member needs.
• Guide staff through complex transitions of care, including hospital discharge, skilled nursing transitions, and housing placement.
• Oversee implementation and coordination of Community Supports services including Housing Transition Navigation, Housing Deposits, Housing Tenancy and Sustaining Services, Medically Tailored Meals, Personal Care and Homemaker Services, and Recuperative Care.
Cross-Functional Communication and Collaboration
• Maintain clear communication with executive leadership and escalate risk issues and barriers in a timely manner.
• Coordinate effectively with billing, intake, operations, and clinical teams.
• Represent Simple Solutions Psychotherapy professionally with managed care plans, county partners, and external stakeholders.
• Contribute to cross-program coordination, including for dual-enrolled members across ECM and CS Housing.
Member Advocacy and Engagement
• Promote trauma-informed, culturally responsive, and person-centered care practices across the team.
• Support staff in addressing barriers to care and improving member engagement.
• Ensure members receive education on health conditions, available services, and community resources.
QualificationsRequired
• Bachelor's degree in Social Work, Human Services, Public Health, Nursing, Psychology, or related field.
• Minimum 3 to 5 years of case management experience working with high-risk or vulnerable populations.
• At least 1 to 2 years of supervisory or leadership experience.
• Experience with Medi-Cal, CalAIM Enhanced Care Management (ECM), and Community Supports (CS) programs.
• Strong knowledge of social determinants of health and community resource systems in San Bernardino and Riverside Counties.
• Excellent leadership, communication, and organizational skills.
• Valid California driver's license, reliable transportation, and proof of auto insurance. LiveScan and TB clearance required.
Preferred
• Master's degree in Social Work, Marital and Family Therapy, Counseling, Public Health, or related field.
• Licensed professional such as LCSW, LMFT, LPCC, RN, or LVN.
• Bilingual or bicultural experience (English/Spanish).
• Experience working with IEHP, Molina, or other managed care plans, or with community-based care coordination programs.

The pay range for this role is:
$38.46 – $47.12 per hour equivalent ($80,000 – $98,000 annualized). Licensed candidates with formal LCSW/LMFT clinical supervision responsibilities: $45.67 – $50.48 per hour equivalent ($95,000 – $105,000 annualized).
