Do you have what it takes to be a Care Coordinator:

• Completion of a Medical Assistant education program or other Certification/Registration preferred
• Experience in Healthcare, primary care preferred

A day in the life of a HSO Care Coordinator:

• Provide telephone, electronic and in-person outreach and consultation to patients who have open orders
• Assist in identifying gaps in care, provide barrier resolution
• Collaborate with healthcare team to optimize patient adherence to medical plan of care
• Educate patient and family members about preventative screenings, identify barriers to care
• Interacts with healthcare team
• Great communication skills telephonically and electronically with accurate messaging to patient and healthcare team
