True Health is seeking a Patient Care Coordinator to be our first in the line of patient service. You’ll connect the dots between patients, providers, and the behind-the-scenes work that keeps care moving.

As a Patient Care Coordinator, you are often the first voice someone hears and the first face they see. You set the tone. You make it easier to get scheduled, easier to get answers, and easier to feel cared for.
What You'll Be Doing
You keep the patient experience running smoothly from first contact through follow up.
You keep schedules accurate and patients on track
· Coordinate appointments, cancellations, and reschedules to support provider availability and patient satisfaction
· Manage appointment requests from the patient portal and route them to the right staff or clinician
· Watch for scheduling issues and help reduce same-day changes and missed appointments
· Communicate scheduling updates clearly so patients and care teams stay aligned
You deliver concierge-level communication and support
· Answer incoming calls and voicemails promptly, pleasantly, and professionally
· Monitor and respond to the office email inbox so messages are addressed within one business day
· Guide patients in accessing and using the patient portal
· Serve as a first point of contact for questions about memberships, billing, and services
You run a welcoming, accurate front desk experience
· Greet patients warmly and help each visit start and end smoothly
· Manage check-in and check-out, including insurance verification and payment collection
· Prepare visit summaries and make sure patient materials are complete and compliant
You keep records clean, organized, and HIPAA-compliant
· Maintain accurate patient records, forms, and agreements with full attention to privacy standards
· File and organize documentation within the EMR so information is easy to find and reliable
· Support audit readiness by keeping documentation complete for patient encounters and incoming contacts
You collaborate across teams to keep care coordinated
· Partner with physicians and clinical support staff on patient needs, scheduling changes, and insurance questions
· Support membership maintenance and renewal tasks as directed
· Coordinate with Lifestyle Medicine, Member Services, and Operations to support patient flow and service delivery

Experience that helps:
· A high school diploma or equivalent
· 2+ years of experience in a medical, healthcare, or concierge office environment
· Proficiency with EMR systems, scheduling, and CRM software
· Experience with patient communication, insurance verification, and basic medical terminology and privacy standards
· An associate degree or higher in Healthcare Administration, Medical Office Management, or a related field (preferred)
· Role involves sustained phone and computer work, extended sitting and talking, occasional light lifting or restocking (10 to 25 pounds), and periodic bending, reaching, and escorting patients or materials within the office.
