About This Position
Description:
Founded in 1866, The Floating Hospital (TFH), is one of the last remaining charity hospitals in New York City. Our commitment is the same as it was more than 160 years ago: unrestricted healthcare to all who walk through our doors, regardless of immigration and insurance status, or the ability to pay. The Floating Hospital welcomes individuals of all ages, from infants, school age children (5+), youth and adults. TFH is a Federally Qualified Health Center and the largest provider to homeless families living in family shelters and domestic violence safe houses throughout all five boroughs.
Essential Functions:
- Liaison between the shelter personnel and Families in Temporary Housing team.
- Ensures shelter information is current.
- Sets up information tables in the shelters.
- Coordinates outreach presentations for residents within shelters.
- Conducts outreach presentations in shelters for case managers.
- Keeps inventory of giveaway items.
- Keeps inventory of outreach materials for shelters.
- Coordinates with other agencies regarding health fairs and other events.
- Works with Case Managers to coordinate clients’ health needs.
- Assists patients during their visit at TFH.
- Participates in homeless policy meetings with other agencies.
- Participates in conferences related to homelessness.
- Provides backup for Transportation to produce pick up schedules.
- Need to be able and comfortable driving in the city.
- Other duties as assigned.
- High school diploma required. Associate’s or Bachelor’s degree preferred.
- Must effectively manage multiple tasks.
- Excellent writing/grammar skills.
- Knowledge of Microsoft Office applications.
- Be available to work in the evenings and weekends as needed.
- Candidate needs to have a clean Driver's License.
