Registered Nurse RN – PRN / Part-Time
Veteran Care Planning, Case Management Support, and Caregiver Training
Location: Field-based position serving clients in their homes
Schedule: Flexible daytime hours, PRN/part-time based on client needs
Work Setting: Non-medical in-home care
Position Summary
Happy to Help Caregiving is seeking a compassionate and detail-oriented Registered Nurse RN to support Veteran clients receiving non-medical in-home care services.
This field-based role focuses on completing in-home assessments, creating VA-compliant care plans, assisting with case management needs, supporting requests for increased service hours when appropriate, and training caregivers on client-specific care needs.
The ideal candidate is an RN who enjoys working independently, supporting Veterans and older adults, and helping caregivers provide safe, consistent, and respectful care in the home.

Key Responsibilities
- Client Assessments and Care Planning
- Complete in-home nursing assessments for new Veteran clients.
- Create individualized care plans using agency templates and VA-compliant documentation standards.
- Document client needs, care goals, visit frequency, safety concerns, interventions, and risk-prevention strategies.
- Identify changes in client condition, care needs, or home safety concerns.
- Ongoing Client Support and Case Management
- Complete routine in-home check-ins with assigned Veteran clients.
- Review and renew care plans every six to twelve months, or sooner if client needs change.
- Communicate changes in client condition, care needs, or service recommendations to agency leadership.
- Support requests for increased service hours when a client’s needs change or increase.
- Help ensure each care plan remains accurate, practical, and aligned with the services being provided.
- Caregiver Training and Support
- Provide hands-on caregiver training related to each client’s care plan.
- Train caregivers on safe transfers, shower assistance, fall-prevention practices, Hoyer lift use, and other client-specific support needs.
- Help caregivers understand how to follow the care plan safely and consistently.
- Serve as a supportive clinical resource for caregivers in the field.
- Documentation and Compliance
- Maintain accurate and timely documentation in the agency’s electronic records system.
- Communicate professionally with clients, families, caregivers, and agency leadership.
- Support compliance with agency policies, VA requirements, and applicable care standards.
- Requirements
- Active, unencumbered Registered Nurse RN license in the applicable state of practice, or compact license if accepted in the service area.
- Strong assessment, documentation, and communication skills.
- Reliable transportation and willingness to travel to client homes within the assigned service area.
- Ability to work independently in a field-based role.
- Comfortable training and coaching non-medical caregivers.
Experience with Veterans, older adults, home care, home health, case management, or care planning is preferred.
Schedule and Travel
This is a flexible PRN/part-time role based on client needs.
Daytime availability is required for in-home visits.
No evenings or weekends unless otherwise arranged.
Travel is required to client homes within the assigned service area.
Travel time and a travel bonus are included for qualifying field visits based on agency policy.
