Physical Therapist at Infinity Care Services Inc – Stockton, California

Infinity Care Services, Inc. Home Health & Hospice Care is looking for a Physical Therapist (PT) to join our team of care providers in San Joaquin County for the TRACY/MOUNTAIN HOUSE AREAS. As a Physical Therapist, you will be delivering the highest quality patient care, according to the specific orders of each patients physician. Responsibilities of a Physical Therapists but not limited to :

Assess patient status to develop and implement a rehabilitation program consistent with the physician's plan of treatment. Work and communicate effectively in a multi-disciplinary team (Physicians, nurses, other therapists) Oversees, evaluates and educates the performance of Physical Therapy Assistants in implementing physical therapy services Patient health promotion and teaching that prevents the onset, symptom and progression of impairments, functional limitations and disabilities that may result from diseases, disorders, conditions, and/or injuries Training of patient family members, as needed Requirements:

Degree from an accredited Physical Therapy Program (approved by the APTA) Current and unrestricted California State License as a Physical Therapist Minimum of 1-year working as a PT in a hospital or nursing home Home Health experience as a PT (preferred) Current CPR certification Current valid state driver's license Must be willing and able to commute to patient home to provide care
