We are seeking an experienced Operations Coordinator to oversee core clinical operations and workflows for Medicare-certified home health.

Key Responsibilities:

• Proficient in Homecare Homebase (HCHB) EMR and workflow manager
• Manage AI‑assisted review of OASIS, coding, care plans and visit plans
• Fully control clinician scheduling to ensure operational efficiency
• Manage all patient and family communication after intake
• Oversee supplies and DME ordering

Qualifications

• Active RN license
• Strong HCHB and home health operations experience
• Detail‑oriented with excellent organizational and communication skills
• Able to multitask
