Assists physician and health care team members in understanding the significant social and emotional factors related to the health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; and participates in discharge planning and in-service programs and acts as a consultant to other agency personnel.

Duties & Responsibilities

• Evaluates the social and emotional needs of patients related to health problems.
• Participates in the development of the plan of treatment.
• Assist patients and their families with personal and environmental difficulties which interfere with obtaining maximum benefits from medical care.
• Participates in discharge planning and in-service programs.
• Maintains expected productivity standards.
• Comply with agency policies for documentation.
• Participates in agency functions required by regulatory and accrediting agencies and agency policies.

Qualifications

• Master’s Degree from a school of social work accredited by the Council on Social Work Education and licensed by the S.C. State Board of Social Work Examiners as a Licensed Baccalaureate Social Worker, Licensed Master Social Worker, or Licensed Independent Social Worker.
• One year of social work experience in a health care setting.
• Professional Certifications.
