Intake RN at South County Hospital – Narragansett, Rhode Island

Receive & process inquiry & referral information including patient clinical data & MD orders. Communicate patient information to IDT members & scheduling department as it relates to the intake & admission of new patients. Conduct discharge planning rounds to medical facilities to coordinate the transfer of information in the intake & admission process of new patients.

Must have valid RI nursing license (diploma, ADN, BSN), BSN preferred. Minimum of (2) years nuring experience in home health care; home care case management experience preferred. Valid driver's license; auto liability insurance as mandated by the State of RI. Must maintain professional liability insurance.
