About This Position

About the Role:

As a Licensed Practical Nurse (LPN) at Concordia Senior Living, you will play a crucial role in providing high-quality care to our residents in a skilled nursing facility. Your primary responsibility will be to assist in the daily living activities of residents while ensuring their comfort and safety. You will collaborate with a multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident. This position requires a compassionate approach, as you will be building relationships with residents and their families to promote a supportive environment. Ultimately, your efforts will contribute to enhancing the overall quality of life for our senior residents.

Minimum Qualifications:

  • Current and valid LPN license.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.

Preferred Qualifications:

  • Experience working in a skilled nursing facility or long-term care setting.
  • Certification in geriatric nursing or related specialty.
  • Proficiency in electronic health record (EHR) systems.

Responsibilities:

  • Administer medications and treatments as prescribed by physicians.
  • Monitor and document residents' health status and report any changes to the nursing staff.
  • Assist residents with daily activities such as bathing, dressing, and eating.
  • Provide emotional support and companionship to residents and their families.
  • Participate in care plan meetings and contribute to the development of individualized care strategies.
