About the Role: Extended Care Registered Nurse RN

In this rewarding after-hours, on-call role, you will:

• Triage and manage after-hours patient needs via phone and onsite visits
• Conduct hospice informational visits and nursing admission assessments
• Support patients and families by collaborating on individualized Plans of Care
• Work alongside an interdisciplinary team to educate, advocate, and provide holistic, compassionate care
• Deliver exceptional service while making a real impact during the end-of-life journey
