Certified Peer Support Specialist- Hospital Discharge at Catawba Valley Healthcare – Hickory, North Carolina

The Certified Peer Support Specialist (CPSS) is primarily stationed at Catawba Valley Medical Center (CVMC), where they play a vital role in supporting individuals transitioning from inpatient psychiatric care to outpatient services. The primary focus of this position is to ensure continuity of care by facilitating successful connections to community-based follow-up appointments and reducing gaps in service delivery.
In this role, the CPSS will conduct individualized assessments to identify patients' needs and connect them with essential community resources — including food assistance, housing support, and transportation services — bridging the gap until patients are successfully established with their follow-up provider. Additionally, the CPSS will facilitate therapeutic and supportive groups for the current inpatient psychiatric population at CVMC.

Position Requirements:
Must be a North Carolina Certified Peer Support Specialist
Preferred 2 years Case Management experience
Preferred experience working with Severe Mental Illness and Substance Use populations
Must have valid Driver’s License and auto insurance
Must have a good driving record
Strong communication skills (written and verbal)
