Certified Peer Support - Hospital Discharge at Catawba Valley Healthcare – Hickory, North Carolina

This Certified Peer Support Specialist (CPSS) is primarily based at Catawba Valley Medical Center (CVMC) to facilitate a smooth transition from the inpatient Psychiatric setting to an outpatient setting by ensuring the community based follow up appointments are kept. This position will assess and assist the patients in connecting with other needed community resources (Food, Housing, Transportation etc) until they are able to connect with their follow up Provider. The CPSS will also facilitate groups to the current inpatient population in the hospital.

Position Requirements:
Must be a North Carolina Certified Peer Support Specialist
Preferred 2 years Case Management experience
Preferred experience working with Severe Mental Illness and Substance Use populations
Must have valid Driver’s License and auto insurance
Must have a good driving record
Strong communication skills (written and verbal)
