At Essential Services LLC, we are more than a provider agency. We empower individuals with intellectual and developmental disabilities by connecting them with qualified Direct Support Professionals (DSPs) who provide personalized, in-home and community-based support.

Our Service Coordinators drive the coordination, communication, and care that make these partnerships successful and meaningful.

About the Role

This is a full-time, in-office position, Monday through Friday from 9:00 AM to 5:00 PM.

As a Service Coordinator, you are responsible for matching clients with the right Direct Support Professionals (DSPs) and maintaining those working relationships to ensure services run smoothly. You will work directly with clients, families, and DSPs to coordinate schedules, support onboarding, and problem-solve in real time to keep care consistent and reliable.

Key Responsibilities

Coordinate & Communicate

  • Serve as the primary point of contact for clients and DSPs
  • Coordinate initial client & DSP introductions and ongoing communication
  • Maintain accurate schedules to ensure continuity of care

Support Clients & Families

  • Provide responsive, respectful customer service
  • Conduct outreach and help resolve service needs
  • Support new client onboarding

Keep Systems Organized & Accurate

  • Maintain client digital folders and documentation
  • Ensure documentation meets agency and compliance standards
  • Perform data entry and administrative support tasks

Collaborate Across Teams

  • Work closely with Service Coordinator Leads and Supervisors
  • Partner with HR to support DSP hiring and staffing needs
  • Coordinate training needs with the Training Department

Problem-Solve in Real Time

  • Arrange shift or ride coverage when DSPs are unavailable
  • Identify service gaps and help implement solutions quickly and professionally

Requirements:

Minimum Qualifications

  • 21 years of age or older
  • Successfully pass a DHS criminal background check
  • Eligible to work in the U.S.
  • Valid driver’s license and insurance
  • No record of substantiated abuse/neglect or CMS exclusion list status
  • Strong written and verbal communication skills
  • Computer proficiency
