Responsibilities

• Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop individualized care plans.
• Administer medications, treatments, and interventions following physician orders while monitoring patient responses.
• Coordinate discharge planning and facilitate smooth transitions from hospital or clinic to home-based care environments.
• Perform diagnostic evaluations and manage acute or chronic conditions with a focus on patient safety and comfort.
• Collaborate with multidisciplinary teams to ensure effective case management, utilization review, and adherence to healthcare protocols.
• Educate patients and their families on disease management, medication administration, and health maintenance strategies.
• Maintain accurate medical records in compliance with HIPAA regulations and ensure documentation reflects quality patient care delivery.

Qualifications

• Valid RN license with current registration in the state of Tennessee
• Critical care experience preferred
• Experience working within Level I Trauma Centers or Level II Trauma Centers is highly desirable.
• Strong knowledge of hospital medicine, infectious disease care, and post-acute outpatient settings.
• Familiarity with medical terminology, physiology concepts, and inpatient/outpatient nursing procedures.
• Proven ability to perform patient assessments accurately and efficiently in diverse clinical environments such as PICU or Geriatrics units.
• Excellent communication skills for patient education, discharge planning, and interdisciplinary collaboration.
• Knowledge of HIPAA compliance standards and medical records management practices.
