Job Title(s): Therapist (LCSW or ACSW) and LCSW Intake Therapist
Salary: LCSW: $44.50 - $46.50 per hour DOE
ACSW: $35.50 - $37.50 per hour DOE
LCSW Intake Therapist: $100,000 per year
Location: Therapist (LCSW or ACSW): Fountain Valley, CA
LCSW Intake Therapist: 1 day at Fountain Valley, 1 day at Garden Grove, 3 days at Tustin

Therapist (ACSW or LCSW) Position Purpose: To provide mental health services, including individual and group therapy, to patients using evidence-based practices and an integrated health approach.
LCSW Intake Therapist Position Purpose: Under the supervision of the Clinical Manager, the Intake Coordinator is an essential member of the clinical team as a facilitator of clinical mental health assessments. The Intake Coordinator’s primary function is to facilitate the admissions process which includes risk assessments and appropriateness for services (both therapy and psychiatry).
Therapist (ACSW or LCSW) Core Duties and responsibilities, include but are not limited to:
- Provide individual, group, and family psychotherapy utilizing evidence-based practices
- Provide offsite group facilitation
- Establish rapport with patients, and establish trust and safety in the therapeutic space
- Complete intake assessments (including PHQ-9, GAD-7, ACEs, etc.) and complete biopsychosocial to gather necessary information for patient care
- Complete treatment plan collaboratively with patients using SMART goals and implement appropriate and relevant treatment interventions
- Conduct ongoing assessment of patient progress in meeting treatment goals
- Facilitate discharge planning and provide necessary linkages for further support, as needed
- Respond to crisis situations and intervene therapeutically as necessary, including assessing for safety of patient, suicidality, homicidally, and self-harming behaviors
- Able to clinically conceptualize patients’ behaviors based on needs and strengths
- Able to incorporate patient choice into treatment model
- Refer patients to outside resources and support as appropriate and needed
- Follow all safety protocols and maintain patient confidentiality
- Collaborate within multidisciplinary team to ensure whole-person treatment is implemented
- Maintain current and acceptable standards of charting and documentation within agency policy of 72 hours
- Adhere to 75% direct service hours
- Ensure all necessary Board of Behavioral Sciences regulations are met, including CEU requirements, to maintain registration up to date
- Exercise independent judgement in clinical matters within scope of practice and if applicable, licensure
- Actively participate in clinical supervision and consultation, including individual and group supervision until full licensure is obtained
- Other duties as assigned
LCSW Intake Therapist Core Duties and responsibilities, include but are not limited to:
1. Provides clinical assessment to determine appropriateness of services using screening questionnaires and risk assessments
2. Coordinates the scheduling of intakes for therapists and psychiatry providers
3. Conducts record reviews (ROI, custody paperwork, etc.)
4. Consults with Clinical Director, Manager, or Supervisor as needed
5. Reviews internal referrals weekly, conducts outreach to referrals within 5 business days, documents attempts, and closes the loop by informing providers of outcome of referral
6. Addresses urgent referrals within 2 business days and closes the loop by informing providers of outcome of referral
7. Communicates with administrative team to ensure review of patient’s insurance and eligibility coverage and informs the patient of copay/good faith estimate
8. Provides individual and group therapy, as needed
9. Ensures all appointments are properly created, scheduled, and confirmed
10. Manages waitlist and waitlist outcomes
11. Attends meetings as applicable to the job title. (i.e. all staff meetings)
12. Behaves professionally and works collaboratively with co-workers to increase productivity
13. Refers therapy and psychiatry patients to outside specialists or agencies as needed, using clinical judgment and higher level of care parameters
14. Responds to crisis situations and intervenes therapeutically as necessary; assesses for safety of patient and assesses for suicidality, homicidally, and self-harm ideation
15. Completes documentation within organization policy of 72 hours
16. Assists with quality measures
17. Provide clinical supervision to ASW and MSW interns
18. Performs other duties and projects as assigned by the Clinical Manager of Mental Health

Therapist (ASCW or LCSW) Education, Qualifications, and Experience:
- Master’s degree in Social Work or Marriage and Family Therapy
- Registered with the California Board of Behavioral Sciences
- LCSW License (In CA)
- LMFT (in CA) preferred
- BLS/CPR certificate
- Ability to efficiently manage time and work under pressure
- Knowledge of Electronic Health Record programs
- Adherence to established legal and ethical codes of the California Board of Behavioral Sciences

LCSW Intake Therapist Education, Qualifications, and Experience:
- Minimum 3 years in related experience
- 4 years post licensure required
- Minimum 2 years of clinically supervising an associate and/or intern
- Experience in clinical intake and assessment processes
- Strong computer skills, acute attention to detail, confident and professional communication.
- Knowledgeable in general EMR/EHR systems is preferred
Work Schedule:
- FTOC is an in-person organization first, and foremost. Employees are expected to be on-site for their scheduled shifts.
- Hours of operation are Monday to Friday 8 a.m. to 8 p.m., however, employee schedules vary, depending on organizational, staffing, community, and patient needs. As such, FTOC may need to modify work schedules to meet such needs.
- Holidays and weekends may be required depending on an employee’s department due to organizational, staffing, community, and patient needs as FTOC continues to grow and expand work days and hours.
- Overtime may also occur due to organizational, staffing, community, and patient needs.
