About This Position

Upstate Family Health Care Inc. (UFHC) is a growing Federally Qualified Health Center (FQHC) in Utica, NY, is seeking board- certified or board-eligible specialists to provide outpatient specialty consultation services on a per-diem / 1099 contract basis. We are looking for the following specialties: Endocrinology | Cardiology | Gastroenterology | Orthopedics | Pulmonology

The opportunity is ideal for specialists who want to maintain a flexible schedule while working in a high-efficiency outpatient clinic environment supported by an established primary care network.

Key Highlights
✔ One dedicated clinic day per week
✔ Outpatient-only practice
✔ No hospital call or inpatient responsibilities
✔ Strong internal referral pipeline from UFHC primary care providers
✔ Expected 20–23 patients per clinic session
✔ Dedicated RN/LPN/MA clinical staff
✔ Experienced medical scribe support
✔ Minimal administrative burden

Our clinic model allows specialists to focus on patient care while our clinical staff and dedicated scribe team manage documentation and workflow, creating an efficient and physician-friendly environment.

Referral Collaboration
Specialists may receive referrals for procedures or advanced diagnostics that are outside the scope of UFHC outpatient services and require hospital or specialty facility care. Such referrals will be made when clinically appropriate and in accordance with patient choice and UFHC referral policies.

Optional Opportunities
Depending on specialty interest and patient demand, physicians may also have the option to participate in:
• Additional clinic sessions
• Telehealth specialty consultations
• Development of specialty programs such as:
o Diabetes and endocrine care
o Pulmonary disease management
o Cardiac risk management
o Musculoskeletal and orthopedic care programs

Participation is optional and based on physician availability and program development.

Compensation
Competitive hourly compensation is structured as a 1099 independent contractor arrangement, based on specialty and experience.

Qualifications
• MD or DO
• Board Certified or Board Eligible in specialty
• Active New York State medical license
• DEA registration (if applicable)

Clinic is open Monday – Friday 8:00am-6:00pm.

Work Environment / Physical Demands
• Ability to stand and walk for extended periods
• Ability to lift/ carry up to 60 pounds and assist patients as needed
• Must be able to bend, stoop, crouch, kneel, twist, and perform repetitive clinical and administrative tasks
• Blood Borne Pathogen Category (OSHA) – Category I – Job related tasks involve exposure or potential exposure to blood, body fluids, or tissue and tasks are a condition of employment
• Must establish effective working relationships, maintain confidentiality, communicate clearly, follow written/oral instructions, and perform duties with accuracy and professionalism.
