As a Care Manager, you will play a pivotal role in delivering high-quality, person-centred care to both potential and existing clients. You will lead and coordinate all aspects of client care, managing the overall client experience while working closely with our dedicated office team to ensure seamless service delivery.
Based in our Blackburn office, you will work with a local client base and regularly visit clients in person, allowing you to build meaningful relationships and deliver care that is genuinely tailored to each individual. Youll have the time and capacity to focus on quality outcomes, advocacy, and the best interests of your clients while supporting the achievement of our business goals.

Key Responsibilities
- Plan, coordinate, and review client care plans to ensure goals and needs are consistently met.
- Work collaboratively with clients, families, and internal team members to develop personalised care solutions.
- Establish service agreements with clients and families, clearly outlining care services and associated costs.
- Maintain accurate and timely documentation of all client interactions and share relevant information with the team, including feedback on caregiver performance.
- Conduct face-to-face meetings with clients and families to review care plans, discuss progress, and identify opportunities for improvement.
About you
- Tertiary qualification in a health-related field is desirable (e.g. social work, allied health).
- 1+ years of experience working in a similar role within aged care.
- Strong organizational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure.
- Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
- Must hold a valid Australian Drivers License and be comfortable travelling as required.
