About the Behavioral Health Care Manager
The Behavioral Health Care Manager (BHCM) serves as an integral part of the primary care team, establishing a therapeutic relationship with patients who meet criteria for enrollment into the Integrated Behavioral Health program. The BHCM provides comprehensive, person-centered, community-focused interventions for enrolled patients. In collaboration with the primary care provider and the psychiatric consultant, the BHCM coordinates the physical and mental health care plans for an assigned patient caseload.

Duties include but are not limited to:
- Supports the development of and communicate the goals for Integrated Behavioral Health services.
- Coordinates physical and mental health care for patients on an assigned caseload.
- Evaluates and ensures program development and growth, including financial sustainability.
- Screen and assess patients for common mental health and substance abuse disorders.
- Provide patient education about care plan needs and available treatment options.
- Coordinates referral patterns and community resource use for the service of the patient population.
- Ensures “warm handoff” delivery and care coordination at all levels of care.
- Track treatment response and monitor patients throughout provision of care. Communicate response to care to primary care providers and psychiatric consultants in weekly case reviews.
- Support medication management with a focus on adherence monitoring, side effects, and effectiveness of treatment.
- Evaluates and utilize behavioral health registry within EMR. Utilize registry and clinical decision support to identify new cases, as well as articulate progress with known patients.
- Document telephone, in-person, and consultative care encounters within the EMR.
- Facilitate treatment plan changes as needed for patient care, in collaboration with both primary care provider and psychiatric consultant. Utilize weekly case reviews to gauge progress and seek additional case review times, as necessary.

Requirements:
Qualifications, Education, and Knowledge
Education: Bachelor’s Degree in a related field, including mental health counseling, marriage or family therapy, social work, nursing, or psychology. Licensure in good standing in the field of service.
Experience: 1. Evidence of additional training in behavioral health or integrative services.
2.1-2 years providing service to behavioral health populations.
3. Knowledge of care coordination in a large health system
- Attendance required at assigned CHDC site(s)
- Must have own vehicle for daily travel in assigned service region.
- Must maintain and provide proof of valid driver’s license.
- Must maintain own automobile coverage as required by law.

Our History
Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we’ve grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient’s medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
