Wellness Equity Alliance (WEA) is a novel national public health organization comprised of a multidisciplinary team of population and public health experts with backgrounds in infectious disease, public health, emergency medicine, primary care, cardiology, pediatrics, psychiatry, community health work (CHW), nursing and advanced practice pharmacy. We work nearly exclusively with underrepresented communities, fundamentally addressing health-care disparities and the social determinants of health (SDoH) that have been amplified during the COVID-19 pandemic, prioritizing the following:

JOB DESCRIPTION
Purpose of the position
WEA has launched a unique street medicine program featuring mental health, substance use disorder treatment, and infectious disease in South Los Angeles and Riverside!
The most vulnerable people experiencing homelessness have likely been failed by institutions many times in their lives, and their mistrust of authorities, institutions, and individual care providers may represent an attempt at self- protection that, over time, becomes a barrier to accessing care and resources that could improve their lives. The street medicine team will work to build trusting relationships with people who are in need of medical services, work to decrease the logistical barriers that block access to health care and provide care directly to the places where unhoused individuals live.
The Project Manager will oversee the delivery of comprehensive, person-centered care planning and coordination services to eligible populations, ensuring compliance with state regulations and alignment with our mission to improve health outcomes for the communities we serve.
Key Responsibilities
- Implement and manage the ECM program in accordance with the CALAIM Enhanced Care Management Policy Guide, ensuring all services meet state requirements.
- Supervise and train ECM staff, including care coordinators and support personnel, to deliver high-quality, comprehensive care management services.
- Collaborate with healthcare providers, social services, and other community partners to facilitate integrated care and support for ECM clients.
- Monitor care plans that are person-centered, addressing the unique needs and preferences of each client.
- Ensure the program meets all documentation and reporting requirements set by the state and federal agencies.
- Analyze program data to evaluate effectiveness, identify areas for improvement, and implement evidence-based strategies to enhance service delivery.
- Manage care management documentation system including data quality metrics and reporting.
- Participate in stakeholder meetings and serve as the primary liaison between WEA and local/state agencies regarding ECM program matters.
- Manage the program budget, including tracking expenditures and optimizing resource allocation for maximum impact.
Qualifications and Education Requirements
- Bachelor’s degree in social work, public health, healthcare administration, or related field.
- Minimum of 3 years of experience in care management.
Preferred Skills
- In-depth knowledge of the claim enhanced care management policy guide and related regulatory requirements.
- Strong leadership and team management skills, with the ability to inspire and motivate staff.
- Excellent communication and collaboration skills to effectively work with diverse stakeholders.
- Proficient in data analysis and program evaluation to drive continuous improvement.
- Ability to travel within Southern California to meet with clients, staff and partners as needed.
- At least 1 year in a supervisory or management role.
