Job Overview:

The BA Clinician Case Manager is a vital member of the Hospital Violence Intervention Program HVIP, a community-based initiative dedicated to reducing violence and supporting victims of violence within our community.

This position provides comprehensive case management services to patients identified through the HVIP, focusing on assessment, intervention, advocacy, and connection to community resources.

The BA Clinician Case Manager works collaboratively with hospital staff, community partners, and other members of the HVIP team to ensure coordinated and effective care for clients.

Qualifications:

Required:

  • Bachelor's Degree in Social Work, Counseling, Community Health or related mental health discipline
  • Commitment to working with clients impacted by violence with compassion and empathy for underserved populations
  • Knowledge and understanding of urban issues, hospital procedures, community resources
  • Strong verbal and written communication and organizational skills to effectively work and communicate with clients, service providers, hospital staff and TRAC Team
  • Proficient in Micrrosoft Office products such as Word, Excel, PowerPoint, Outlook
  • Punctual and reliable
  • Able to work in stressful situations and problem solve, including verbal de-escalation of clients

Preferred:

  • Bilingual in English and Spanish
  • 1-2 years providing case management and/or clinical services in a fast-paced environment
  • Clinical experience working with behavioral health population, crisis intervention, and case management in a hospital setting

Scheduling Requirements:

  • Shift working, Tuesday-Saturday 9am-5pm; 1 shift a week 7am-3pm Tuesday or Thursday
  • Full-Time (FT)

Essential Functions:

  • Stationed in the Emergency Department twice a week or as needed to assess patients for TRAC eligibility; engage potential patients at bedside and/or outreach patients telephonically for full evaluation of needs and safety planning
  • Actively recruit and enroll eligible patients into the Violence Intervention Program, meeting or exceeding established enrollment targets
  • Respond to referrals from hospital staff regarding patients who have experienced violence and ensure outcomes are promptly reported to Director for tracking purposes e,g,, assault, domestic violence, community violence
  • Provide intensive case-management, advocacy, and support to victims of violence for medical/mental health treatment, social service supports, financial assistance, victim services, linkage to criminal/civil legal advocacy, and transportation services
  • Following up on these services and referrals is a primary responsibility of this position; along with tracking successful connection to resources
  • Provide accompaniment to appointments, court hearings, and other relevant meetings as needed
  • Works collaboratively with the team and provides support when needed to meet program expectations and ensure quality of service to patients
  • Vets and maintains an up-to-date knowledge of available community resources and eligibility requirements to build community supports related to client s needs
  • Conduct TRAC intake interviews and assessments, create and manage service plans, updating them as needed
  • Document assessment and client contacts in electronic medical record consistently and accurately; complete all documentation within prescribed timeframe and maintain appropriate records as required by program and hospital policy, and applicable accrediting organization standards,
  • Write in a professional and competent manner
  • Collaborate and maintain strong relationships with hospital staff, community partners, and other stakeholders to ensure coordinated and comprehensive care for clients
  • Participate and represent the HVIP program at community events and health fairs 1-2 a month to raise awareness and promote services
  • Participate in data collection on patient demographics, services provided, and outcomes
  • Prepare regular reports on program activities and outcomes such as monthly patient success stories
  • Maintain compliance with all organizational and departmental policies and procedures
  • Attends staff meetings, workshops, trainings, and supervision
  • Other responsibilities as assigned by management
