About This Position

**Licensed Practical Nurse- Rate of Pay: $40.57 per hour - Weekend Differential
Pay $41.39 per hour

*****2nd Shift PT 24 hours - 3pm - 11pm Every Other Week/ Days Vary
Scope of Service: The Licensed Practical Nurse (LPN) works under the supervision of the Director of Nursing, (APRN) / Director of Health and Wellness, (RN) providing technical skilled nursing care, medication administration as needed to clients in their homes or other appropriate settings in accordance with agency policies and procedures. The LPN promotes, maintains, and restores the health of individuals and the community through appropriate teaching and rehabilitative measures. Must possess good communication skills and be able to work in a variety of environmental settings. The position must present a professional appearance at all times while maintaining strict confidentiality of client information. Performs other duties as assigned.

ESSENTIAL JOB FUNCTIONS:
Exercises appropriate nursing principles, procedures, and techniques used in
the care and treatment of the clients served.
Provides direct and individualized nursing care to clients based on the nursing
standards and under the supervision of the Director of Nursing and/or Director of
Health and Wellness.
Ensures quality and safe delivery of technical skilled nursing to clients, families,
caregivers.
Assists Advanced Practice Registered Nurse (APRN) and /or Registered Nurse
(RN) in developing the client care plan. Identifies client needs for other health
care services, makes referrals at request of APRN/RN
Regularly communicates client's progress to registered nurse, staff, and
physician.
Carries out physician's/APRN orders as written in the plan of care. Teaches and
assists client according to the plan of care.
Implements plan of care formulated by RN, APRN, MD, while following medical
directions and service policies.
Manages assigned caseload, maintaining records and writing reports according
to agency policies within Agency’s time frames.
Provides accurate and timely documentation consistent with plan of care.
Maintains safe and healthy work environment and uses safe work methods and
procedures in accordance with agency’s clinical standards and policies.
Reports and documents adverse reactions to medications, as well as unusual
activity, behaviors as directed by MD/APRN.
Supports programs in working with clients to address comorbidities and provides
resources and education as necessary.
Conducts medication reconciliation daily and as needed; manages accuracy of
the Medication Observation Record and/or Medication Administration Record;
transcribes medication orders as needed and reports any and all concerns to
Director of Nursing, Director of Health & Wellness, Program Supervisor.
Coordinates with staff to arrange for further medical attention and follow-ups
when needed.
Implements natural, holistic, and medical supports in treatment process, Per
APRN/MD order.
Provides education for clients and staff regarding medical and psychiatric care
with emphasis on teach-back method.
Works with staff/clients providing education regarding preventative care and
healthcare alternatives.
Consults as part of the interdisciplinary team.
Provides crisis intervention and support as clinically indicated.

Environmental Responsibilities:
Assists in maintaining a safe and secure environment for staff and clients.
Maintains a clean, well-organized work environment.
Notifies supervisor of maintenance and/or safety issues and generates work
orders as needed.
Conducts and participates in fire and emergency evacuation drills, and reviews
the emergency preparedness plan as outlined in agency policy.
Completes other designated environmental tasks as assigned.

Quality Assurance:
Adheres to policies and practices, including medication practices as outlined in
staff handbook and program operations manual.
Works in a respectful, constructive, and cooperative manner with fellow staff and
providers.
Upholds all Health Insurance Portability and Accountability Act (HIPAA)
regulations and secures all necessary releases of information for outside
communication.
Models culturally competent behavior interacting with clients,
professionals/providers, and fellow staff.
Maintains accurate records and case files as assigned and in accordance with
programming, contractual, licensing, and accreditation requirements.
Adheres to the standards of the Council on the Accreditation of Rehabilitation
Facilities.
Participates in agency committee work when available or as permitted.
Completes all trainings as required per program contract and agency policy.
Attends agency sponsored in-services, outside trainings, conferences and
meetings to facilitate continued professional growth and ensure the quality of case
management practices.

QUALIFICATIONS
CT LPN license required with a minimum Associate Degree in nursing or related
field.
Preferred two years’ experience in Psychiatric Nursing in social services settings.
Prior experience working with people with mental illness and substance use
disorders is required.
Valid Connecticut Driver’s license is required. Reliable transportation and
willingness to transport clients in personal vehicle that hold valid liability
insurance.
First Aid/CPR certification or willingness to obtain certification within 90 days of
hire. Employee is responsible to maintain ongoing certification.
Annual physical, PPD, and other health requirements as required by Continuum
and the Dpt. Of Public Health.
Proficient computer skills required, including Microsoft Office and electronic
record keeping.
