Nurse Manager II – UM / Discharge Planning at Bright Vision Technologies – San Francisco, California

We are seeking an experienced Nurse Manager II – Utilization Management (UM) / Discharge Planning to lead care coordination and patient flow initiatives in a fast-paced acute care hospital environment. This role is ideal for a strong clinical leader with expertise in case management, utilization review, and discharge planning.

• Lead daily operations of UM and discharge planning functions
• Oversee patient care coordination and ensure smooth transitions of care
• Manage and develop case management and nursing teams
• Ensure compliance with regulatory standards (Joint Commission, Nursing Practice Act)
• Monitor KPIs related to patient flow, length of stay, and quality outcomes
• Collaborate with physicians, care teams, and external providers
• Drive process improvements to enhance efficiency and patient outcomes
• Manage staffing, resource utilization, and departmental performance
• Support strategic planning and operational initiatives

• Bachelor’s Degree in Nursing (BSN) required
• Active California RN License
• BLS Certification
• 5+ years of patient care experience
• 4+ years of leadership experience
• Strong recent experience in Utilization Review (UR), Case Management, or Discharge Planning in an acute care hospital
