Position Summary
-
Under general direction, protects, promotes, and optimizes health and abilities. Prevents illness and injury. Alleviates suffering through the diagnosis and treatment. Advocates for patients and their families. Ensures all actions taken in carrying out responsibilities support patient centered care.

Responsible for comprehensive goal-oriented care of patients as directed by the physician orders. This includes patient assessment, nursing diagnosis, care planning, implementation, intervention and evaluation of care. Works with an interdisciplinary team to assure support of the patients’ physical, spiritual and emotional needs, as well as the needs of the families.

- Collaborates with providers, palliative nurses, outside facilities, social workers, and case
managers to identify potential hospice patients. When appropriate, continues collaboration
to obtain referrals, hospice orders, perform patient intake, and assist in arrangement of
discharge and potential placement. - Serves as patient case manager at hospital bedside. In conjunction with the Interdisciplinary
Care Team (IDT) uses assessment information to develop and implement the
interdisciplinary plan of care, focusing on the patient’s goals, identified problems, and
palliation of symptoms. Reevaluates the patient’s condition and ongoing needs, and reports
patient’s condition to appropriate members of the IDT. - Conducts referral and admission visits at bedside in the hospital, in outside facilities, and in
patients’ homes as assigned. - Provides bereavement education and support, and encourages use of bereavement services.
- Educates patients, families, hospital staff, outside facility staff, new hospice team members,
and the public about hospice services and end-of-life care. - Assists with chart audits and data
collection when requested.Participates in Hospice and MGMC committees and work groups. - Assists with MGMC’s Transitional Care program.
• Practices exemplary interpersonal skills while advocating for patients and communicating
with patients, families, providers, and other staff members. -
Qualifications, Knowledge & Experience

Required Qualifications (Including any licensure, certification, education):
-
Licensed as a Registered Nurse and authorized by the State of Iowa to practice nursing or has active multistate license approved by the Nurse Licensure Compact.

Achievement of Baccalaureate degree in nursing within 6 years of hire if hired after January 1, 2014.
-
Basic Life Support (BLS) training, or within 90 days of hire and must maintain throughout employment.

Mandatory Reporter training or within 90 days of hire and must maintain throughout employment.

Two years experience in medical surgical nursing.
-
Possession of a current valid driver’s license and must maintain eligibility for coverage under Mary Greeley Medical Center’s insurance policy.

Preferred Qualifications:-
Baccalaureate degree in nursing.

Professional certification in nursing specialty.

Required Knowledge, Skills & Experience:-
Proficient in communication skills.

Ability to pursue and foster a sense of team and partnership across all disciplines.

Ability to communicate effectively, in English, with diverse population groups.

Ability to be self-directed, flexible, and able to adapt to change.

Ability to communicate effectively in a professional and tactful manner with physicians, staff, patients and/or their representatives.

Ability to read and understand chart documentation.

Ability to demonstrate basic computer skills.

Ability to read, write and understand the English language.

Knowledge of theory, techniques, and practices of professional nursing in caring for neonatal through geriatric clients; knowledge of preparation and administration of medicines; knowledge of treatments and medical procedures; knowledge of counseling and teaching techniques; knowledge of the preparation and operation of appropriate medical equipment; ability to make independent decisions.
