About This Position

Position Summary

The Program Specialist/Supervisor oversees 1 or more Residences and related Direct Care Staff and/or one or more Life-Sharing homes. This position also manages Direct Care Staff providing non-residential home-based services. Much of the work associated with this position includes: Program development; Individual monitoring and evaluation; as well as the supervision of Direct Support Professionals. Program Specialists/Supervisors can also work with specific Family Living Providers (FLPs) or manage the agency’s Life-Sharing program.

Compliance

  • Ensures that Individual to Staff ratios are always maintained in accordance with relevant regulations.
  • Ensures that all Direct Support Professionals are properly trained and certified to distribute medications to Individuals.
  • Ensures that FLP homes are compliant with all relevant State, County, and Regional requirements.
  • Schedules staff coverage for all sites under their supervision.
  • Responsible for the development and signing of all annual FLP contracts with the Agency.
  • Attends Individual Support Plan (ISP) meetings.
  • Reports changes pertaining to Individual needs to the relevant SC.
  • Provides ISP review documentation to the SC and plan team members as required by state regulations.
  • Provides direct supervision for Direct Support Professionals.
  • Attends a minimum of 24 hours of in-service training per year.
  • Becomes certified as a Medication Administration Trainer and ODP Certified Investigator.
  • Other duties as needed.

Financial Stewardship

  • Supports and oversees Individual finances, ensuring that Individual funds are secure and within the Individual’s care and control.
  • Responsible for monitoring all Individual spending and reviewing relevant financials and documentation, including all information necessary for social security redetermination.
  • Other duties as needed.

Programming

  • Performs direct consultation/casework with Individuals to facilitate their optimum development.
  • Identifies Individual skills and strives to support their on-going development and growth.
  • In conjunction with the Program Director, participates in the development of ISPs.
  • Recommends revisions to the ISP as warranted.
  • Informs all Laurel House plan team members of all ISP meetings.
  • Responsible for the accurate and responsible implementation of Individual ISPs.
  • Assists in the planning and preparation of Agency-based programming initiatives.
  • Identifies and coordinates Individual participation in community events that are of Individual interest.
  • Responsible for completing all data forms, accountability forms, and other required reports.
  • Acts as a liaison between the Agency and relevant County and State Agencies.
  • Serves as an Advocate for Individuals to assure rights and choices are safeguarded.
  • Supervises, monitors, and evaluates services provided to the Individual.
  • Counsels Individuals on personal problems and values and offers guidance on making informed decision.
  • Serves as the primary liaison between Individuals, their families, and other significant people in the lives of Individuals.
  • Transports Individuals as needed.
  • Assumes on-call responsibilities as scheduled.
  • Coordinates the training of Direct Support Professionals and the family in the content of the health and safety needs relevant to each Individual.
  • Other duties as needed.

Human Resources

  • Primary liaison between all Direct Care Professionals and Program Director/Human Resources/CEO.
  • Assign household maintenance and purchasing tasks to Direct Support Professional, overseeing their performance to ensure the meeting of Individual needs.
  • Leads regularly scheduled house or staff meetings to address the overall delivery of care.
  • Responsible for all Direct Support Professional staff work schedules.
  • Conduct all on site Direct Support Professional and related training.
  • Attend a minimum of 24 hours of annual training.
  • In the absence of other Program Supervisors/Specialists, assumes their duties, responsibilities, and authority.
  • Participate in all internal incidents.
  • Other duties as needed

Essential Functions

  • Must be able to push/pull/lift up to 50 pounds.
  • Must be able to sit/stand/walk for extended periods of time.
