About This Position
The Case Manager for the Crisis Stabilization Unit (CSU) at Southern Highlands Community Mental Health Center (SHCMHC), a CARF-accredited Certified Community Behavioral Health Clinic (CCBHC), provides intensive, short-term care coordination and support to individuals experiencing behavioral health crises. This position focuses on rapid assessment, stabilization, and transition planning to ensure continuity of care and reduce the need for higher levels of intervention.
The Case Manager works collaboratively with multidisciplinary teams to develop and implement person-centered, trauma-informed treatment plans. Responsibilities include coordinating access to behavioral health, medical, social, and community-based services; advocating for clients; and facilitating safe and effective discharge planning. This role requires the ability to respond to crisis situations, support individuals with complex needs, and promote recovery, stability, and community integration.
Education and Experience:- Bachelor’s or Master’s degree from an accredited college or university in a human services-related field (in accordance with West Virginia Board of Social Work requirements)
OR - Licensed Psychologist with a Master’s or Doctoral degree from an accredited program
OR - Licensed Registered Nurse
Experience in crisis services, behavioral health, case management, or acute care settings is strongly preferred.
