Location: Rancho Mirage, CA and surrounding cities.

Job Description Summary:
The Hospice RN Case Manager (field) plans, organizes and directs hospice, and plans and directs services to meet the needs of patients and their families within their homes and communities.

Essential Job Functions/Responsibilities:
• Completes an initial, comprehensive and ongoing assessment of patients.
• Initiates the plan of care and makes necessary revisions as needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
• Acts as Case Manager and assumes responsibility to coordinate patient care.
• Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver self-care techniques as appropriate.
• Attends interdisciplinary group meetings.
• Fulfills the obligation of requested and/or accepted case assignments.

Position Qualifications:
• CA RN License
• Compassionate & Caring
• HomecareHomebase (Pointcare) Experience
• CPR certification
• CA Driver License

Schedule:
• Weekdays
• 8 hour shift

Experience:
• Hospice nursing: 1 year (Required)
• Homecare Homebase EHR (PointCare/HCHB) : 1 year (Preferred)

License/Certification:
• California RN License (Required)
• BLS/CPR (Required)
• California Driver License (Required)
