Job Summary:

We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) Charge Nurse to join our skilled nursing facility team. The ideal candidate will be responsible for overseeing nursing staff, ensuring high-quality patient care, and maintaining a safe and supportive environment for residents.

Key Responsibilities:

- Supervise and coordinate the activities of nursing staff to ensure effective patient care.

- Assess and monitor patients' health status, documenting changes and communicating with the healthcare team.

- Administer medications and treatments as prescribed by physicians.

- Develop and implement individualized care plans in collaboration with the healthcare team.

- Provide education and support to patients and their families regarding health management and care plans.

- Ensure compliance with facility policies, procedures, and regulatory requirements.

- Participate in staff training and development to enhance nursing skills and knowledge.

- Address and resolve any patient or family concerns in a timely and professional manner.

Skills and Qualifications:

- Current LVN license in the state of practice.

- Previous experience in a skilled nursing facility or similar healthcare setting preferred.

- Strong leadership and supervisory skills.

- Excellent communication and interpersonal abilities.

- Ability to work effectively in a team-oriented environment.

- Proficient in electronic health records and other healthcare technology.
