About the Role:
As a Physical Therapist with VNA, you’ll provide in-home rehabilitative services to patients recovering from illness, injury, or surgery. You’ll also offer palliative therapy to our hospice patients, helping enhance their quality of life and independence. You’ll work closely with an interdisciplinary team of RNs, PTAs, OTs, SLPs, MSWs, and Home Health/Hospice Aides to ensure coordinated, patient-centered care.

What You’ll Do:

• Deliver hands-on physical therapy care per individualized treatment plans

• Assess mobility, gait, strength, range of motion, and equipment needs

• Educate patients, families, and caregivers on therapeutic techniques

• Provide palliative PT for hospice patients, supporting comfort and dignity

• Supervise and guide Physical Therapist Assistants (PTAs)

• Collaborate with physicians and interdisciplinary care teams

• Document care in compliance with Medicare, state, and agency standards

• Support performance improvement initiatives and maintain compliance

What You’ll Bring:

• Bachelor’s or Master’s in Physical Therapy from an accredited program

• Current PA Physical Therapist license (or application in process)

• Minimum 2 years of experience; home health experience preferred

• Valid PA driver’s license, reliable transportation, and auto insurance

• CPR certification
