Job Overview:

Provides Social Work evaluation and treatment to patients receiving care through the Craniofacial Center; educates patient and patient families regarding their plan of care; makes referrals to other providers when appropriate; provides recommendations at Craniofacial Center Team Meetings, provides final evaluations to the Craniofacial Center Coordinator within 30 days of evaluation.
