Job Title: Intake Coordinator (Part-Time, 20–25 hours/week). Schedule is flexible; however, candidates must be available to work Mondays, Fridays, and Saturdays.
Job Summary: The Intake Coordinator is responsible for managing the intake process and assisting with the admission process for hospice patients. This role ensures a smooth transition by coordinating referrals, verifying eligibility, obtaining necessary documentation, and facilitating communication between referral sources, patients, families, and interdisciplinary team members.
Job Qualifications:
Education: High school diploma required; associate’s or bachelor’s degree in healthcare administration, nursing, or a related field preferred.
Licensure: Current Driver’s License.
Experience: Prior experience in hospice, home health, or medical intake coordination preferred.
Skills: Excellent organizational and multitasking abilities. Strong communication skills, both verbal and written. Ability to work in a fast-paced environment and manage multiple referrals at once. Ability to handle sensitive information with discretion and confidentiality. Knowledge of Medicare, Medicaid, and private insurance hospice regulations.
Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions:
Works in a routine office environment. Noise level may be moderately high, the ability to work a flexible schedule and extended hours. The ability to travel locally and some exposure to inclement weather. Possible exposure to toxic materials, toner, etc. Requires the ability to handle stressful situations in a calm manner.
Essential Functions:
· Receive, process, and coordinate hospice referrals from hospitals, physicians, nursing facilities, and other sources in a timely manner.
· Verify patient eligibility for hospice services, including insurance benefits, Medicare/Medicaid coverage, and physician certifications.
· Gather and review medical records, physician orders, and other required documentation to ensure compliance with hospice admission criteria.
· Communicate with patients, families, and referral sources to provide education on hospice services, answer questions, and address concerns.
· Coordinate with the hospice team, including nurses, social workers, and physicians, to schedule initial assessments and admissions.
· Maintain accurate and up-to-date documentation of all referrals, admissions, and communication in the electronic health record (EHR).
· Ensure compliance with Medicare, Medicaid, and other regulatory guidelines related to hospice admissions.
· Collaborate with marketing and community outreach teams to strengthen referral relationships and promote hospice services.
· Identify and address any barriers to admission, such as insurance authorizations or missing documentation.
· Maintain confidentiality and adhere to HIPAA regulations when handling patient information.
· Perform quality control checks on all intake documentation to ensure accuracy, completeness, and compliance with hospice regulations.
· Perform other tasks and duties as assigned by the Admissions Director.
