Job SummaryUnder the supervision of the PACE Clinic Manager and under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members.

Responsibilities
Job SummaryUnder the supervision of the PACE Clinic Manager and under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members.
- Receives and communicates appropriate information to staff and keeps Clinic Manager informed of changes, needs, complaints, etc.Responds to emergencies involving participants.Participates in the Quality Assurance program by conducting audits and submitting results in writing.Assists in the implementation of new procedures.Performs functions related to clinical assessment, care planning, evaluation of participant care.Teaches participants and families based on identified needs of participant.Documents nursing care in the medical record.Able to triage participants in clinic effectively.On call responsibilities.Administers medications and maintains medication system including over the counter, contingency and emergency medication supply participants under clinic standards of practice and within scope of licensure.Completes transcription and follow-up of medical orders.Performs phlebotomy procedures as needed.Maintains cleanliness and organization in the clinic, especially in the medication area and cart.Participates and functions as a member of the interdisciplinary team and rotates as needed.Independently delivers nursing care and services to program members within scope of licensure and per primary care orders or nursing care plan.Schedules and coordinates med pass and monitors for quality review of medication errors.Mentors and assists in the orientation of new clinic staff hires.In collaboration with the purchasing agent, oversees medical, pharmaceutical and oxygen supplies in the clinic. Tracks assignment of oxygen concentrators and other Durable Medical Equipment as indicated.Scheduling and managing appointments/assessments for MDs and Nurse PractitionersWound care and maintaining wound records/documentationDaily nursing tasks, preparing clinic for patients to be seen by provider.Assist with bedside procedure including gynecological exams, phlebotomy.Maintaining participant safety while at the centerAct as liaison between providers/family/pharmacy and will be an active member of interdisciplinary team and cross train to home care RN position.Ability to maintain excellent and respectful working relationship with co-workers and customers from diverse backgrounds.
Minimum Qualifications
- Bachelor's in Nursing DegreeCurrent California RN license and good standing with the Board of Registered Nurses Current CPR certification which includes in person certification and test on resuscitation models Minimum three year experience in acute care nursing, outpatient clinic or medical office Experience in triage (in-person and via telephone)Licensed as a Registered Nurse in the state of employmentBLS certification required.
- PACE experienceOne-year experience as a Registered Nurse; with an elderly population
- Must be able to read, write, speak and understand the English language.Must possess the ability to make independent decisions when circumstances warrant such action.Must possess the ability to deal tactfully with staff, members, visitors, government agencies/personnel and the general public.Must be knowledgeable of dietary procedures.Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult members.Requires valid driver’s license.
- $120,000 $130,000 Depending on Experience
