Neurology Nurse Navigator

JOB SUMMARY

This position provides clinical support for neurological clients including; education, communication, and care coordination with other healthcare professionals to improve client outcomes. Collaboration with those affected by Neurological conditions to enhance their adaptive and coping skills, and facilitated empowerment and a sense of control. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.

PRIMARY CLINICAL JOB RESPONSIBILITIES

  1. Facilitate and support the management of symptoms and problems that occur in relation to varied Neurological conditions.
  2. Enhance and promote safe, maximal function.
  3. Support wellness-focused quality of life.
  4. Triage patients by phone for medication refills, test results, and questions, soliciting orders/direction when appropriate from the physicians. Communicates back with the patient and documents appropriately in the medical record.
  5. Providing education and support to patients and their families to include:
    • Diagnosis
    • Medication- risks and benefits
    • Side effect management
    • Symptom management
  6. Communicate with patients via phone regarding test results and instructions about treatment plans per physician instruction.
  7. Coordinates multidisciplinary care including referral process and exchange of information and records; prior authorization for medications and other treatments
  8. Coordination of and assistance with the disability process.
  9. Advocate for access to services on behalf of the patient.
  10. Ensure that necessary appointments are scheduled in a timely fashion.
  11. Performs nursing office visits including nursing assessments, vital signs, and medication reconciliation. Documents findings in the medical record and alerts physician to significant abnormalities.
  12. Assist with monitoring for accreditation readiness and office quality improvement initiatives.
  13. Consents and assists the physician with in-office procedures.
  14. Administers and documents oral, IM, subcutaneous, and dermal medications as ordered by the physicians.

DEPARTMENTAL AND ADDITIONAL JOB RESPONSIBILITES

  1. Assist office staff with prior authorizations.
  2. Assist office staff with FMLA paperwork and other patient disability paperwork needs.
  3. Other duties as assigned by managers and physicians.

JOB SPECIFICATIONS

EDUCATION

  1. Current Registered Nurse (RN) with licensure in the State of Michigan is required.
  2. BSN is required, MSN is preferred.

EXPERIENCE

  1. Two years of recent RN experience in the specialty area of Neurology is preferred.
  2. Certification as CNRN Nurse is preferred or willing to obtain within 2 years of hiring into this position but not required.

ESSENTIAL PHYSICAL ABILITIES/MOTOR SKILLS/SENSORY REQUIREMENTS

These physical requirements are not exhaustive and additional job related physical requirements may be added on an as needed basis. Corrective devices may be used to meet physical requirements.

  1. Sedentary Work: Frequently required to stand, kneel and crouch. On a daily basis may be required to move about, sit, climb stairs and bend. Is frequently required to lift and carry up to 20 lbs. May be required to push/pull/carry items between 20-100 lbs.
  2. Vision: Requires the ability to perceive the nature of objects by the eye. Near acuity: Clarity of vision at 20 inches or less. Midrange Acuity: Clarity of vision at distances of more than 20 inches and less than 20 feet.
  3. Motor Coordination: While performing the duties of the job, it is required to regularly perform functions that include using hand and finger movement, handle or feel objects, be able to use tools or equipment that requires reaching with hands and arms. Must be able to travel independently throughout hospital; access patients/families including areas confined by space and/or equipment.
  4. Speaking/Hearing: Ability to give and receive information through speaking and listening skills.

ESSENTIAL TECHNICAL ABILITIES

  1. Able to navigate computer systems and enter data into database.
  2. Use of Microsoft Office products.

  1. Able to problem solve. Identifies and resolves problems in a timely manner. Gathers and analyzes information skillfully.
  2. Analytical, Prioritization and planning of work activities.
  3. Attention to detail and accurate documentation of research and office activities.

  5. Ability to manage multiple priorities.

  1. Can apply effective interpersonal skills to provide service. Comfortable working in a team environment. Adapts to changes in the work environment.
  2. Demonstrates professional conduct and communicates in a professional manner with patients, physicians, office employees, and other hospital personnel.
  3. Demonstrates knowledge and support of Memorial Healthcare mission, vision, and value statements, standards, policies & procedures, confidentiality statements, and corporate compliance plan.
