Position: Bilingual Case Manager
Classification: Full-time, Exempt
Reports to: Program Manager

Position Description:
The Case Manager will provide a broad range of case management services to children in substitute care and the families with whom they are placed. Primary responsibilities include prioritizing and managing crisis frequently; working one on one with client, working with larger groups such as families, staff groups and focused task groups; maintaining a high load of telephone communication; and handling after-hours on-call when scheduled.

Primary Responsibilities:

  • Provide intake and assessment activities that will assist clients and their family in determining which services are most appropriate for them in developing an individualized service /treatment plan.
  • Utilizing client and family input, develop a plan of service that selects and outlines an array of services and interventions from within DePelchin and the community that will address the needs formulated in the assessment process.
  • Assist clients with referrals to other community services identified in the plan of service and provide advocacy for individual clients and their families to receive entitlements or needed services.
  • Provide support services in anticipation of discharge and post-discharge services that are most appropriate for them to aid the transition to a new placement, track outcomes for youth in care, support normalcy.
  • Support the family and client post-discharge through Aftercare service delivery.
  • Provide crisis management support for children and families by implementing interventions that support de-escalation.
  • Complete quarterly inspections for assigned homes.
  • Maintain contact with foster families, to ensure compliance.
  • Follow all agency/department/program policies and procedures and comply with the legal and regulatory requirements of the Council on Accreditation, Youth for Tomorrow, Residential Child Care Licensing, Residential Child Care Contracts and other appropriate regulatory bodies.
  • Enter relevant information/changes and renewable information into Solutions.
  • Serve as liaison to TDFPS Residential Child Care Licensing for sampling visits and other related inspections for the foster/adoptive home.
  • Ensure all documentation is entered into electronic health record. Monitor frequently to ensure facility and/or client compliance.
  • Maintain current child record meets all recording requirements of the agency.
  • Provide monitoring of foster home specific to children’s plan of service and supervision needs as well as compliance with the physical home in accordance with Minimum Standards.
  • Coordinate treatment by consulting and collaborating with referral sources, teachers, school counselors, and other professionals involved with the client.
  • Meet productivity expectations by maintaining assigned caseload and managing schedule to meet appropriate timeliness.

Required Qualifications:

  • Bachelor’s degree, preferably in social work, or other human services field.
  • Two (2) years of full-time documented experience in child welfare services conducting assessments, service planning, case management, and crisis management preferred.

Knowledge, Skills, and Abilities:

  • Knowledge of Social Work practices.
  • Must have excellent written and oral communication skills.
  • Ability to maintain professional, confidential work environment.
  • Ability to work with culturally diverse populations and work force.
  • Ability to work flexible hours which vary according to client needs.

Work Conditions:

  • Environment: Hybrid - Home-based, Community Settings
  • Range of Schedule: Mon – Fri, 7:00 a.m. to 8:00 p.m.
  • Travel: Frequent, Local (Must have reliable personal transportation and valid Texas Driver
