As a Care Partner, you will play a key role in delivering exceptional care experiences for both new and existing clients. You will work closely with clients and their families to understand their goals and develop tailored care plans that support their independence and wellbeing.

Key Responsibilities
- Plan, coordinate, and review client care plans to ensure goals and needs are consistently met.
- Work collaboratively with clients, families, and internal team members to develop personalised care solutions.
- Establish service agreements with clients and families, clearly outlining care services and associated costs.
- Maintain accurate and timely documentation of all client interactions and share relevant information with the team, including feedback on caregiver performance.
- Conduct face-to-face meetings with clients and families to review care plans, discuss progress, and identify opportunities for improvement.
About you
- Qualification in a health-related discipline such as social work, allied health, nursing, or a related field.
- At least 2 years' experience in a similar role working within aged care.
- A compassionate and empathetic approach, with a genuine passion for supporting seniors and improving their quality of life.
- Strong organisational skills with the ability to effectively manage time and competing priorities.
- Ability to work in a fast-paced environment and adapt to changing priorities.
- Excellent written and verbal communication skills.
- A valid Australian Drivers Licence.
