We are looking for experience for Health Practitioner/Health Coach/Community Health Coordinator to support our US team.

Location: Onsite/Residing Philippines
Employment Type: Full-time/Independent contractor
Duration: Long term
Responsibilities:

• Act as a primary non-clinical point of contact for patients
• Conduct wellness check-ins, follow-ups, and patient engagement activities
• Provide basic health education, lifestyle guidance, and wellness coaching (non-diagnostic)
• Perform community or field-based coordination, including local follow-ups and support as needed
• Assist patients with scheduling, referrals, and navigating health or wellness services
• Collect patient information and escalate clinical concerns to the Registered Nurse
• Maintain accurate documentation and follow escalation and compliance protocols

Qualifications:

• Background in health-related fields such as nutrition, allied health, public health, wellness, or community health
• Strong communication and interpersonal skills
• Willingness and ability to perform community or leg work when required
• Ability to follow clinical direction and escalation guidelines
• Comfortable using digital tools for documentation and communication

Preferred Qualifications

• Certification or training in health coaching, nutrition, wellness, or community health
• Experience working with barangay health programs, NGOs, or community outreach initiatives

• Multilingual skills (Filipino, English, or regional dialects)
