Job Description
High School Diploma or GED required
- Assisting with prospective resident queries and admissions procedures.
- Screens all prospective admissions to determine that the delivery of appropriate care will be provided in the long term care setting.
- Organizing and inputting information to databases.
- Handling phone calls and emails from prospective residents, families, and visitors.
- Planning and coordinating admissions with all interdisciplinary departments.
- Assesses cost-of-care of prospective admissions appropriate to the level of care as determined by the admitting physician.
- Screens all prospective admissions to determine that the delivery of appropriate care will be provided in the long-term care setting.
- Performs routine networking visits to physicians, hospital discharge planners, case managers and other healthcare referral sources to present center information, gather resident referrals, and evaluate potential resident admissions that contribute to the center’s overall revenue growth; Maintaining relevant records and documentation as required by the job.
- Explains costs of care involved in appropriate levels-of-care as determined or required by the admitting physician.
- Participates in census development and marketing meetings, conference calls and planning meetings as scheduled.
- Performs other duties as assigned.
- Serves as liaison between families and hospital discharge planners, social service agencies, and physicians.
- Maintains list of community resources.
- Attends and participates in Quality Assurance meetings.
- Providing input on how to improve processes based on resident and referral source feedback.
- Assesses cost-of-care of prospective admissions appropriate to the level of care as determined by the admitting physician.
- Performing other work-related duties like project planning and general administration.
