About This Position

Job Summary:

The Patient Care Coordinator (PCC) is accountable and responsible for managing the clinical care and course of treatment of a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, nurses and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The PCC possesses the knowledge and skills necessary to provide care appropriate to the age, cultural and religious beliefs of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The PCC is accountable to the Unit Director and will function as an extension of the Director with oversight for the practice of nursing.

  • Ability to effectively communicate with patients, physicians, and co-workers in a manner consistent with a customer service focus and application of positive language principles.

    Provides for exceptional patient and coworker experiences by following our standards of practice of always using positive language; promotes a positive work environment.

    Responsible for making patient assignments and monitoring breaks and lunches; as well as making sure patient ratios are safe

    Evaluates performance of assigned staff

    Makes rounds in each area every 2 hours and as needed

    Completes the PCC checklist

    Attends Discharge Planning Meetings during the week

    Provides written report for oncoming PCC

    Completes OTIS as needed during shift

    Reviews and/or assists with all discharges

    Holds meetings with staff as needed on any updates from Director

    Resolve daily problems and patient care concerns independently

    Notifies Director of any concerns or unresolved complaints immediately

    Assist nurses in documentation and picture taking of wounds from admission and throughout admission as needed

    Completes accurate and timely documentation

    Keeps a clean, safe, quiet, and efficient work environment

    Serves as a resource person for nurses and other staff on the unit

    Reviews all patients charts throughout shift to ensure all specimens have been collected, restraint documentation is correct as well as relevant care plan information

    Provides an initial assessment on admission for any admits for the LPN as well as initiate the care plan

    Administers IV push medications as needed for LPN, or anything else not within their scope of practice

    Is able to manage staff as well as the clinical care and course of treatment of multiple patients

    Able to safely utilize equipment including, but not limited to: Defibrillator, Automated Blood Pressure Machine, Hoyer lift, tele medicine, restraints, wound vac, blood warmer, SCDs, Bair hugger, Omni-cell

    Completes the control substance count weekly at night or as needed

    Assist with home oxygen DME setup, as well as home health, when case management is not in house

    Able to care for the cardiac patient and identify lethal cardiac dysrhythmias

    Ability to assist with set up and maintenance of sterile field during procedures

    Ability in competency of central line care, IV insertion/care and blood transfusions

    Demonstrates the ability to communicate effectively with staff, managers, physicians and all other members of the hospital

    Plans patient care in conjunction with other disciplines to meet the goals and individualized needs of the patients

    Competent in patient interventions as evidenced by safe administration of medications, treatments, procedures, infection control, and addressing changes in status to safely care for patients Evaluates the practices of care delivered to patients and adjusts care according to established standards of practice and physician’s orders

    Educates staff, patients and family on equipment, medications, safety, pain control, and discharge needs

    Logs/ addresses needs for specialized beds

    Demonstrates the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to safety, security, emergency preparedness, control of hazardous materials, life safety, and utility systems as outlined in the hospital’s environment of care plan

    Utilizes knowledge and provides care of age specific needs of patients, from pediatric to adult in performance of duties and responsibilities

Qualifications:

Education/Skills

Graduate of a recognized accredited non-online RN program.

Work Experience:

Minimum of two years as a staff RN in an acute setting; six months experience at Highland Community Hospital preferred.

Certification/Licensure-DUE UPON HIRE

    Licensed RN able to practice within the State of MS
