We’re looking for a bold, compassionate Family Nurse Practitioner to join our Metro Detroit team and help deliver meaningful, patient-first care in a DPC environment.

What You’ll Do

Clinical Care & Patient Relationships

• Serve as a primary care provider for a dedicated patient panel, building long-term, trusted relationships.
• Examine patients, obtain comprehensive health histories, and assess physical and behavioral health needs.
• Diagnose and manage acute and chronic conditions with a focus on prevention and continuity of care.
• Prescribe medications and therapies in collaboration with supervising physicians, as required.

Diagnostics & Procedures

• Order, perform, and interpret diagnostic testing (labs, EKGs, imaging coordination).
• Perform in-office procedures including injections, suturing, wound care, and immunizations.
• Provide proactive follow-up and care coordination to reduce unnecessary ER visits and specialist referrals.

Holistic & Preventive Care

• Counsel patients on lifestyle, nutrition, chronic disease management, growth and development, family planning, and preventive health.
• Deliver culturally competent care to a diverse patient population, including Spanish-speaking members.
• Partner with patients to create personalized, goal-driven care plans.

Team Collaboration

• Work closely with physicians, medical assistants, and care team members to ensure seamless care delivery.
• Support clinic operations by supervising and coordinating support staff when needed.

What You’ll Bring

Required

• Active Michigan Family Nurse Practitioner (FNP) license
• Master’s Degree in Nursing or related field
• Bilingual in English and Spanish
• Strong clinical judgment, patient advocacy mindset, and commitment to relationship-based care

Preferred

• 2–5 years of experience in Primary Care
• Experience in outpatient, community-based, or value-based care models
• Comfort working in a fast-paced but patient-focused clinical environment
