About This Position

Position Overview:
The special role of "Care Access Pro" (CAP) is the heart of what HealPros does. In this role, you will be able to travel to people's homes and help them access care like Diabetic Retinopathy Screenings, Bone Density Screenings, A1C test kits and more. Many of the people our team serves are unable to go to appointments, so bringing care directly to them means you can:
1. Save someone from going blind
2. Contribute to their overall health and wellness
3. Engage with people who might be lonely and need someone to talk to
4. Alert their health plan to potential issues you observe while in their homes
5. Travel to new places and build lasting connections!
All Care Access Pros attend HealPros University prior to seeing members in the field, which includes a virtual class and live classes at our headquarters in Atlanta, and the opportunity to ride along with an experienced CAP to see how a typical appointment works. Key Responsibilities:
• Travel to health plan members’ homes to bring access to care to individuals who may have difficulty attending traditional appointments
• Perform non-clinical health screenings and support services, primarily Diabetic Retinopathy Screenings, and when applicable, bone density screenings and assistance with at-home test kits, following HealPros training and protocols
• Capture required images and information using HealPros-provided equipment and technology
• Guide members through screening processes and test kit completion without providing medical advice or clinical interpretation
• Observe and document relevant non-clinical information during home visits and escalate findings to HealPros or the member’s health plan as required
• Engage with members in a compassionate, respectful manner, including individuals who may be isolated or underserved
• Complete multiple scheduled in-home appointments per day (typically 5 to 10), each lasting approximately 30 minutes
• Travel within a defined service area, primarily within a 90-mile radius of home, with occasional extended travel as needed
• Successfully complete required courses through HealPros University prior to performing field work

Required Skills and Qualifications:
• Kind, compassionate, and people-centered approach to working with culturally diverse members
• Strong interpersonal and communication skills, with the ability to put people at ease in their homes
• Comfort working independently in a field-based, travel-oriented role
• Ability to follow structured workflows and protocols in a non-clinical capacity
• Willingness and ability to travel regularly between appointments
• Valid Driver’s License and access to a reliable personal vehicle
• Basic comfort with mobile technology, apps, and digital tools provided by HealPros
• Dependable, organized, and able to manage time effectively in a schedule-driven environment
• Background or interest in healthcare, patient support, community outreach, or mobile services (clinical licensure not required)

Physical Requirements
• Ability to travel independently between scheduled in-home appointments, including driving for extended periods and navigating residential environments
• Ability to perform the physical activities necessary to complete contracted services, including standing, sitting, bending, and moving within members’ homes
• Ability to transport and handle equipment and supplies required to complete in-home screening services
