RN Home Care, Full-time
Under the direction of a Home Care Agency Director RN, provides home care patient care activities and accommodation services in accordance with standards, regulatory guidelines and licensure requirements. Maintains compliance with Agency policies and procedures.
Essential Duties and Responsibilities:
- Follows established Standard of Nursing Practices and implement Facility policies and procedures.
- Knowledgeably and competently performs nursing skills.
- Monitors and manages the departmental budget in conjunction with the Home Care Director.
- Identifies health and safety hazards and initiates corrective action immediately including assuring the safety of Clients under his/her supervision.
- Supervises and evaluates all direct client care and initiates corrective action as necessary.
- Receives and provides report from/to nurse he/she is relieving/coming on duty and records sufficient information to implement appropriate follow-up as necessary.
- Visits clients in their homes to perform medical assessments, including a complete physical assessment and history of current and previous illnesses. Assesses client needs and initiates development of individualized plans of client care.
- Develops an accurate comprehensive care plan based on each client's assessment that establishes goals based on client need and incorporates therapeutic, preventive, and rehabilitative actions.
Minimum Education:
Graduate of accredited school of nursing. Bachelor’s degree of Science in Nursing is preferred. Current Nursing License (be current). CPR/ First Aid certified (Will be provided if not certified).
Minimum Requirements:
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage.
- CPR/First Aid certification required.
- RN Licensure required.
Minimal Experience:
- A minimum of one year’s experience in working with the frail or elderly population required.
- Minimum of two years’ work experience as a registered nurse required. One (1) year home care experience preferred.
