Director of Operations - Home Health Agency at Five Star Home Health Care – Chantilly, Virginia

Full-Time | Monday–Friday | 100% Office-Based

We are seeking a skilled Director of Operations to oversee unified intake and clinical workflow operations across all service lines. This role ensures patients receive a seamless onboarding experience and that clinicians are fully supported through optimized processes.

Responsibilities:

• Serve as Homecare Homebase (HCHB) EMR Super User
• Lead centralized intake operations across all service lines
• Oversee eligibility verification and authorization processes
• Direct patient assessments to support appropriate care planning
• Ensure accurate payor coverage and benefit determination
• Oversee patient and family education on services and care processes
• Lead after‑hours intake operations and escalation support
• Drive workflow optimization and EMR best practices
• Oversee AI‑supported review of OASIS, care plans, and visit planning
• Manage supplies and DME ordering workflows
• Lead centralized scheduling and patient communication efforts
• Maintain SOPs and lead staff training and onboarding

Qualifications:

• Active RN license
• Extensive home health operations experience
• Strong understanding of payor rules and authorizations
• Super User with Homecare Homebase EMR
• Excellent communication, organization, and leadership skills
