About This Position

Position Title: Physical Therapist – Hospital (2875 West 19th St)

Full-Time

Shift 8AM - 4:30PM

Compensation: $81,120K - $118,518K

We provide Rehabilitation Services, including Physical Therapy, Occupational Therapy, and Speech Therapy, at the main hospital as well as multiple outside outpatient clinics. This full-time, 80% outpatient and 20% inpatient position, offers flexible days and hours, time off and reimbursement for CEUs, licensure renewal, and mentorship for new graduates. We pride ourselves on treating patients 1:1 with no patient overlap for the full 1-hour session. In addition to having an open-door policy, we encourage a good work/life balance.

Position Purpose:

  • Evaluates and treats patients using the principles and practices of physical therapy in accordance with the patient's age, developmental level and diagnosis.

Essential Functions:

  • Evaluates patients, establishes goals and develops individualized treatment plans. 
  • Provides direct patient treatment based on current physical therapy practice and revises the treatment plan based on the patient's response.
  • Provides education to the patient and family regarding patient abilities and the need for assistance and continued therapeutic intervention.
  • Consults with physicians on patient's evaluations, progress, and complications with treatment.
  • Maintains medical and departmental records according to department policies and procedures.
  • Participates in patient care conferences, department meetings and department and hospital in-services.
  • Communicates with other professional staff and patient's family/support system to provide continuity of care. 

General Job requirements:

  • Doctor of Physical Therapy, Bachelor’s degree prior to 2015 accepted
  • Illinois PT license
  • CPR Certification
  • Experience in an outpatient setting preferred, but not required
  • Bilingual English/Spanish a plus, but not required
  • Ability to communicate with staff at all organizational levels
  • Effective written and verbal communication skills
  • Customer service skills
