In this role, you will:

Support patients facing complex social, behavioral, and economic challenges Help remove barriers to care through resource connection and advocacy Work across programs including primary care, prenatal, substance use, and behavioral health Be part of a collaborative, team-based model focused on whole-person care

🧠 What You’ll Do

As a Social Worker – Float, you will:

Connect patients to housing, benefits, and community resources Support patients across multiple programs and service lines Provide compassionate, confidential support in sensitive situations Communicate effectively across in-person, phone, and electronic platforms Document all patient interactions clearly and in a timely manner within the EHR Collaborate with providers and interdisciplinary teams to support patient care

🧩 What You’ll Bring

Required:

Bachelor’s Degree in Social Work or a related human services field Strong ability to maintain confidentiality and professionalism Excellent communication and interpersonal skills Ability to manage multiple priorities in a fast-paced environment

Preferred:

Experience working with vulnerable or underserved populations Experience in healthcare, behavioral health, or community-based services
