Home Health Registered Nurse (RN)
Reports To: Director of Nursing (DON) / Clinical Supervisor
Classification: Part-Time
Location: Field-based / Patient Homes ( DFW, Ft Worth, Denton, Longview/Tyler, Arlington
POSITION SUMMARY
The Home Health Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their place of residence in accordance with the physician-ordered Plan of Care, CMS Medicare Conditions of Participation, and Texas Health and Human Services (HHS) regulations.
The RN conducts Start of Care (SOC), routine, recertification, PRN, and discharge visits, ensuring safe, high-quality, patient-centered care while supporting positive clinical outcomes and preventing avoidable hospitalizations.
ESSENTIAL DUTIES & RESPONSIBILITIES
Clinical Care

Perform Start of Care (SOC) assessments and comprehensive nursing evaluations.

Provide skilled nursing interventions per the physician-ordered Plan of Care.

Conduct routine, PRN, recertification, and discharge visits.

Monitor patient condition and promptly identify and report changes to the physician.

Perform cardiopulmonary assessments, medication management, pain management, and disease monitoring.

Provide post-operative care, wound care, IV therapy, and chronic disease management as ordered.

Accurately measure and document vital signs, weight, edema, wounds, and functional status.
Medication Management
-
Complete medication reconciliation at SOC and throughout the episode of care.

Educate patients and caregivers on medication purpose, dosing, and side effects.

Monitor high-risk medications (e.g., anticoagulants, diuretics, insulin).

Identify and report medication discrepancies or adverse reactions.
Patient & Caregiver Education
-
Provide disease-specific education (e.g., CHF, diabetes, hypertension).
-
Teach fall prevention, infection prevention, nutrition, and symptom management.

Reinforce safe use of medical equipment and assistive devices.
-
Evaluate patient and caregiver understanding and need for reinforcement.
Care Coordination
-
Collaborate with physicians, therapists, home health aides, and interdisciplinary team members.

Communicate effectively with caregivers and family members.

Participate in case conferences and interdisciplinary care planning.

Ensure timely follow-up on physician orders and care plan updates.
Documentation & Compliance
-
Complete accurate, timely documentation in the electronic health record (EHR) per agency policy.

Ensure documentation meets CMS, Texas HHS, and Medicare requirements.

Support agency compliance with audits, surveys, and quality initiatives.

Maintain patient confidentiality and HIPAA compliance at all times.
Safety & Professional Practice

Follow infection control and universal precautions.

Maintain a safe working environment in patient homes.

Practice within scope of nursing practice and agency policies.

Maintain professional licensure, certifications, and required training.
WORK ENVIRONMENT

Patient homes, assisted living facilities, or independent living settings.

QUALIFICATIONS
Required
-
Current Registered Nurse (RN) license in the State of Texas.
-
Graduate of an accredited nursing program (ADN or BSN).
-
Minimum 1 year of nursing experience (home health or med-surg preferred).
-
Knowledge of Medicare home health regulations and standards of care.
-
Ability to work independently in the field.
-
Reliable transportation and valid Texas driver’s license.
-
Current CPR certification (BLS).
-
Ability to pass background check, drug screen, and TB screening.
Preferred
-
Home health or hospice experience.
-
Experience with OASIS documentation (OASIS-E preferred).
-
Wound care, IV therapy, or chronic disease management experience.
-
Familiarity with electronic health records (Healthie or similar).
-
Strong assessment, teaching, and communication skills.
PHYSICAL REQUIREMENTS
-
Ability to travel to patient homes.
-
Ability to lift, push, pull, or carry up to 50 pounds.
-
Ability to stand, bend, squat, and walk for extended periods.
-
Ability to perform patient care tasks in varied home environments.
